ARTHRITIS

Arthritis is a very general term, many times is can simply be degeneration of cartilage after a particular injury or in other cases the loss of cartilage from a chronic disease such as rheumatoid arthritis. We cover some techniques - fusions were the damage is severe of cartilage repair where possible.

COMPLEX WOUNDS

We specialize in healing of difficult wounds , heel and diabetic foot pressure wounds. We always address the root case of the wound as well as close the wound as quickly as possible. We use the patients own skin to achieve this and try to avoid any fake skin grafts. We are one of only a few surgeons in the country that specializes in skin graft and plastic surgery rotational and free flaps and microsurgery in the foot.

EQUINUS

Equinus is the contracture of a joint- in this case the ankle joint. There are many reasons for this but ultimately what happens in the inability to place the heel on the ground properly when walking and loss of range of motion of joint. There are many surgical releases we perform as well as physical therapy and nonsurgical treatments.

Flatfeet can be congenital : developed from birth or acquired in older age where tendons weaken and allow the arch to collapse. we perform surgical reconstruction of the foot using both minimal incision implants as well as complete surgical reconstruction to recreate fallen arches in pes planus flatfoot condition.

REVISION SURGERY

CLUBFOOT

Clubfoot deformity is challenging but very rewarding in terms of how visible the changes to a patient’s life are like. Casting is usually performed when a baby is born with clubfoot and works very well. DROPFOOT

Dropfoot is also called foot drop. This condition can severely affect a persons daily activity and with strokes and blood clots on the rise we are seeing this condition more often than ever before. We perform tendon transfer and nerve regeneration techniques with minimal incision to correct this deformity and correct walking gait patterns.

TARSAL TUNNEL SYNDROME

Dr. Gitlin specializes in nerve repair and decompression surgery as well as nonsurgical treatment with minimal incision for delicate nerve surgery. The technique used was developed by Dr. Gitlin and has been used since 2005. We perform tarsal tunnel and anterior decompression as well as diabetic neuropathy nerve release.

Charcot-marie-tooth affects the ankles in a way that weakens certain tendons and allows other tendons to deform the foot. Usually the patient ends up with a high arch foot ( cavus foot) and a weakness of walking muscles. The reconstructive surgery is very specialized. and a number of different factors must be addressed at the same time the foot is realigned.

NON UNION

Nonunion is what we call when bones do not heal, delayed union is a term used when the bones are not healing as fast as they should. Malunion is where they healed in the wrong position. And finally an infected nonunion is exactly that - and a clinically challenging situation.

Osteomyelitis is bone infection, surgery or trauma complications can cause this, sometimes diabetic foot wounds can also cause bacteria to reach a bone. There are many different treatment from surgery to antibiotics. Patients need to be cared for by someone who specializes in this specifically.

Children can be born with flat feet, but there are a significant amount of people who can develop if later in life. There are nonsurgical and surgical techniques that might be used to treat this condition. This does need to be addressed since if not treated ankle arthritis can develop quickly.

TARSAL COALITION

When joints are forming each cell is assigned what it will be, sometimes cartilage or sometimes bone. In some cases this programming can have an error and the bones will connect in an area where they should not. This is a coalition and is common in certain joints in the foot. PERONEAL TENDON

The peroneal tendon is on the outside of the ankle and its main function is to pull the foot outward. It is common to injure this tendon during an ankle sprain (twisting your ankle). here we cover acute injuries, chronic tears and how this is addressed.

The arch of the foot comes in many shapes. Some are low arches that we call flat feet, others can be higher than the average, this is referred to as a cavus foot type. click to learn more

POSTERIOR TIBIAL TENDON

The posterior tibial tendon is on the inside of the foot and its main function is to pull the foot towards the other foot and to help hold up the arch. It can deal with a significant amount of stress but can also be easily injured for most common acute and chronic injuries.

ACHILLES TENDON

This is a technique of bone surgery that can increase blood flow to a limb that had peripheral vascular disease.

Injuries to the anterior tibial tendon are not so common but i wanted to address this here. This tendon in on top of the foot and its main function is to help pull the foot up an inward. Tears of this tendon need to be repaired in most cases since the patients foot can lose function and become deformed if not done.

In the 1950s the science behind bone lengthening was described. Any bone in a skeleton could be made longer or wider through a certain kind of surgical procedure. We use these techniques to fix deformities in the legs. We can make feet longer and make people taller as well.
